A lot of female athletes do that, here is the thing, you are running in an endurance race, you are downing 200 ml of water every table you pass at a check point, and you are trying to get a gold medal. Stopping to waste valuable time to pee is not an option - and by that point they have probably downed so much water its basically water thats coming out by that point - which is why so many photos are around, she looks wet but there is no smell. Probably doesn't even cross their mind. Some probably got over the embarrasment in their first couple of races. Especially if they scored higher than those who were handicapped by letting it bother them.

Various atheles deal with it different ways, from hyrophobic mesh pants that dont stain when wet, to incontinence pants like astronaughts and aircraft pilots use under the shorts.

Triathalon runners just do it in their skin suit or shorts during the water stages.

Everyone else, the races are so short the need to go probably helps them run faster.
