That happens all too often, but it's usually covered up, at least here in the UK. I had an ex-wife and she worked on a production line, to use the toilet they had to ask the supervisor to get someone to take their position so the line could still run whilst they used the toilet.
The lines were run by mostly younger women, late teens to mid twenties, but the supervisors were male, my ex had several near misses where she'd only just made it to the toilets, then one day a younger girl was kept waiting so long that she actually wet herself.
